Pension applications cannot be faxed or emailed. Pension applications must be delivered in person to the fund office or mailed with the required original documents enclosed. If mailed, required original documents will be returned to the sender’s address.

Applying for Pension Benefits

No more than six (6) months before your pension eligibility date, you should contact the Pension Fund office for an Application to Commence Payment of Pension Benefits.

At that time, you will need to provide originals or certified copies with seal of the following documents:

  • Participant’s proof of age (see acceptable list of documents below)
  • Spouse’s proof of age (see acceptable list of documents below)
  • Marriage Certificate
  • Copies of any divorce decrees or death certificates of any former spouses

Photocopies are unacceptable. Originals will be copied and immediately returned.

Acceptable Proof of Age Documents

Documents acceptable to prove your age are listed below in order of preference. Originals/certified copies are required and will be returned:

  • Official birth certificate from Bureau of Vital Statistics (preferred)
  • Baptismal certificate or statement certified by custodian
  • Census Bureau notification of birth registration
  • Hospital birth record or certificate
  • Foreign church or government record
  • Certification by physician or midwife present at birth
  • Certification of Bible or other family records

If none of the above documents are available, any two of the following may be submitted:

  • Naturalization record
  • Immigration papers
  • Military records
  • Current Passport
  • School records certified by custodian
  • Life insurance policy with application
  • Labor union or fraternal record certified by custodian
  • Marriage license or certificate with full date of birth
